The CSB is deploying investigators to Accurate Energetic Systems after an October blast killed sixteen and caused a huge damage.
The U.S. Chemical Safety and Hazard Investigation Board (CSB) announced it is deploying a team to investigate a fatal explosion that took place on Oct. 10, 2025, at the Accurate Energetic Systems (AES) facility in McEwen, Tennessee. The incident reportedly caused 16 deaths, multiple serious injuries and tremendous damage to the facility. AES produces explosive products for defense and industrial applications.
According to the Bureau of Alcohol, Tobacco, Firearms and Explosives (ATF), approximately 24,000 to 28,000 pounds of explosives detonated, with the preliminary blast taking place in a manufacturing place where explosive materials were heated in kettles. Also explosive materials on the same floor reportedly detonated after the first explosion.
The CSB team will determine the site, which had been beneath ATF control until lately for debris evaluation and removal of undetonated explosives. Investigators will even meet with AES management and federal officers to accumulate operational and facility records had to determine the reason of the incident.
CSB Chairperson Steve Owens stated that, “This is one of the deadliest industrial incidents in our country in years. Our investigators will work to discover the factors that brought about this tragic event and give recommendations to prevent same occurrences.”
The CSB is an independent federal agency that investigates industrial incidents regarding extremely unsafe chemicals. The corporation issues protection suggestions however does not have regulatory or enforcement authority.
